PayWindow. Looks clunky as hell but seems to be accurate on tax rates and has multi-state support, support for multiple hourly rates, accrued leave, etc.

Very easy to set up, prepares your reports for you, and it's relatively easy to use. For about 15 employees I only averaged about an hour a week on payroll, and a couple hours extra a month on submitting witholding taxes and preparing IRS 941 reports. I could never justify spending the money on outsourcing a small payroll when the time involved is that minimal. If it makes you feel better have your accounting firm prepare and double check your year end returns and W-2's. For five employees though, it's just not that complicated, and payroll is an expense that IMO you need to have an intimate knowledge of.
